sort of
legal stuff
do I need to
fill out in
order for my
child(ren)
to attend an
Oasis
event?"
In order
to save
you, the
parent,
some
time
(and
ink),
you only
need to
fill out
the 'Waiver'
and 'Medical
Release'
forms
once per
school
year.
After
filling
out the
'Waiver'
and
'Medical
Release'
forms
once for
a school
year,
you can
simply
fill out
the 'Consent'
form,
which is
a
half-page
form
that
serves
as your
official
seal of
approval,
giving
your
child(ren)
permission
to
attend
specific
Oasis
events...
without
filling
out
multiple
pages of
paperwork.
I hope
the
Oasis'
new
highly
simplified
registration
process
serves

hand,
and your
favorite
writing
utensil!

1)
Please
download
and fill
out the
'Waiver'
and 'Medical
Release'
forms
only
if...
a)
you
have
not
filled
them
out
since
January
15,
2008,
or
b)
your
personal
information
has
changed
since
January
15,
2008,
or
c)
you
have
not
filled
out
these
forms
during
the
current
school
year
(September
1st
-
August
31st).
If none
of the
above
stipulations
apply to
you,
then
simply
skip
this
step
because
you're
good to
go! How
easy!
2)
Please
download
and fill
out the
'Consent'
form,
which
must be
filled
out for
every
student
on a per
event
basis.
This
form
essentially
serves
as your
parental
seal of
approval,
which
allows
your
child(ren)
to
attend a
specific
Oasis
event.
3)
Simply
hand the
completed
form(s)
directly
to Chad
or one
of the
church
secretaries.
4)
You're
done!
Lord
willing,
we'll
see your
child(ren)
at an
Oasis
event!
"Is there
anything
else I need
to know?"
Yes.
Because
of the
incredibly
simplified
event
registration
process,
we need
you, the
parent,
to keep
us
updated
if any
of the
information
on the
'Waiver'
or
'Medical
Release'
forms
should
change
over the
course
of the
school
year
(i.e.
change
of
address,
phone
number,
insurance
information,
allergies,
medications,
etc.) so
that we
are
always
in the
best
possible
position
to
provide
the best
possible
care for
your
child in
the
event of
an
accident.
To
update
this
information,
simply
download
new
forms
and
submit
them to
Chad or
one of
the
church
secretaries.
